Dear Sir/Madam, I have applied for a SC 491/189/190 visa (select the visa type). I need to add my new born baby to this application. Attached herewith are the relevant documents.
Could you please be kind enough to add my (baby age) days old newborn baby to my application together with the medicals request (if required) and provide us our visa grant. Below are our application details:
